Comparison of different sampling or inverse probability weights, and no weighting

The prevalences (%) based on questionnaire variables, and means (and prevalence (%) of obesity) of health examination variables were estimated based on no weighting (“SRS”, complete-case analysis), sampling weights (“Sampling”, not accounting for nonparticipation) and different inverse probability weights (IPW). The estimation was based on the participants of the full QS sample or the HES sample. In the HES sample, estimation was also based on the participation in “any (health) examination or questionnaire” or in “both (health) examination and questionnaire”.

The IPW weighting models labelled “Original” were based on the sociodemographic and hospitalization register data available up to end of 2021, whereas “Pre-sampling predictors” included the follow-up until the sampling date. The extended follow-up contained the variables based on the follow-up data from the sampling date to the end of 2023 (time until the first event).
In addition to the point estimates and their standard errors (SE), the design effects (DEFF, ratio of the variance estimate and the corresponding SRS variance estimate) were also reported.

Variables

From the health examination we selected the following continuous variables: total, HDL and LDL cholesterol (mmol/l); BMI; obesity (binary, based on measured BMI ≥30 kg/m2); and diastolic and systolic blood pressures (mmHg, both as mean of two measurements).

Results

All weights for the QS sample produced similar estimates, but for the HES sample there were larger differences. For the QS sample, the unweighted estimates differed in several variables, such as in difficulties in walking 500 meters, where the oversampling of the 75 –year-olds or older was 11.2% compared to the weighted estimates, which ranged between 6.9% and 7.5%. The prevalence estimate of daily smokers (9.6%) was close to the QS estimates (between 9.7% and 9.9%) for the HES-any when using the original weights based on the limited subset of predictors, but with the extended set of predictors the prevalence estimate was lower (9.0%); the unweighted QS estimate was lower as the prevalence of smokers was low (3.1%) among the 75 year old or older. In the HES-all subset the prevalence estimates were 8.4% and 7.8%, respectively, which were close to the estimates with no weights or sampling weights only. Similar findings were found for several other variables. In the health examination variables, the weights had almost no influence on the results. Design effects were larger in the QS sample, as the sampling probabilities varied between both WSCs and age groups whereas the HES sample was close to a self-weighted sample, thus the design effects were close to one, and the difference of standard errors between the QS and HES results were smaller than indicated by the number of participants.

In the physical measurements of the HES, weights had little influence on the estimates, but there were large design effects in the blood pressure, especially in the oldest age groups, and slightly smaller design effects in the cholesterol, but almost no design effects in the measured BMI.
